Do you also have to repeatedly sign into all apps? Does pressing pause and play on live TV work without any stuttering , freezing or jumping? (They also signs of a faulty hard drive)
Since all of your recordings have been lost it very much appears to be a hard drive issue so your box will need replacing, especially if the other two issues also occur.

As an aside, do you have different profiles set up? If so then the recordings might only be visible via the one profile that set them up.
Also have you tried a factory reset? (it might only be a temporary solution)
To factory reset go to Settings (the cog at the end of the Home menu) > System > Factory Reset. It's actually misnamed and only a partial system reset and not a complete factory reset. You are given 2 options
Keep Recordings
Format Disk
Select Keep Recordings, which means you won't lose your recordings, and any planned recordings, or series links you've set up. You may have to sign into your apps such as iPlayer, Prime, Netflix and ITVX again.
