From what has come out though it does sound like a sudden event. He didn't steal anything, he didn't know her, he apparently had no reason to stab her, and yet he got up and did so on a busy bus. It's madness.

Sudden psychotic episodes are not uncommon. I know a guy convinced a drug dealer moved in next door to get him and he went round trying to kick the door in intent on doing him serious harm
