Has the cfg file for 20Mbit on the 256 at the tftp server been changed for the upgraded one before the upgrade has been done? Faults say there isn't a fault logged in our area (SO30) but it probably is a network fault.

You're locking to a standard VXR CMTS as per your modem remembering the last frequency it locked to and reacquiring, then being kicked over to a next generation CMTS and a DOCSIS 3 bonded group on a 10k or BSR but are unable to lock to it.
Either the next generation CMTS or eQAM is not functioning or the config has been deployed prematurely to your area and there's no next-gen CMTS to sync to yet
