How to address the Prime Minister and military officers.
Does anybody know the definitive way to correctly address the Prime Minister upon first meeting him?
Also, do any former armed forces members know the correct way to address a Military Officer upon first meeting them?
Are there differences depending upon the sex of the person in question?

I would do what Mr Bean did with the Queen for the PM.

A) Prime Minister (optional to attach PM’s surname) - when I was meeting with some of previous PMs, the etiquette was "Prime Minister"
B) "rank" surname, or Sir/Ma’am (eg Captain Rogers)

It does depend on your own status. There is a difference between civilians, armed forces (and emergency personnel).
For non-civilans it would be Sir or Ma'am.

I usually address the current PM as "Pal" since he has a face like a lump of dog meat
