Agreed. Did she not think to tell her staff to actually test the algorithm before approving it? Very careless and unprofessional. Ofqual assured ministers that the algorithm was robust. Were they supposed to disbelieve their advisors? Of course not. She had to go.

They are hired by a recruitment board, not a Select Committee, and the board recommend a candidate to the Secretary.
Something similar happened to me when I went for an IT Director’s position at a University - I was recommended for the position after a gruelling interview process, but I had to meet the Vice-Chancellor, and be approved by him (did he think I would fit in with him, his Senior Team, and the Uni’s "culture"?*).
Same thing happened when I contracted at another Uni as Head of IT Systems Development - after selection, I had to meet the Chief Operating Officer for approval.
In both cases, they had vetoed other candidates, and they had restart the recruitment cycle.
What the stats are for OfQual and it’s ilk, I don’t know, but I would imagine it’s the same process, otherwise you are just wasting the Minister’s time for a box ticking exercise...
* I got the job...
