Some people said it went off for them on normal ITV! Probably a regional thing then.

This is a world wide issue, HD output is controled by FIFA's media centre i.e. ITV buy into the SD/HD media rights from FIFA.
Spain and France blogs have also reported same issue.

He got a yellow card, they took him off to protect him (in case he got a second one, which would be a red, which would see him missing games).

Yes, yellow card. That early in the game knowing Milners attitude there was a high probs that he would get a red
