I have used ViPER Guestbook on some of my sites. It is good only problem is it is no longer supported by its creators ver 1.2 is the final release.

If you are hosted with i-web it is in the Fantastico menu.

Hmm, these things may be good, but if yoyu have a poplar site, put some guards in against the spammers who'll love to get their dodgy web addy on the search bot lists.
