They look like you're on an extremely long/poor quality line, and that it's either set to prioritise speed over stability or that it's rather unstable as well.
With your terrible attenuation (due to the long line) I can't see there being much that you can do.
Do you at least use the master socket, rather than an extension? Have you ever tried the hidden test socket (behind the master's faceplate) to see if that makes any difference?
I'd probably ask for a higher SNR margin, the current one (if it's what I think it is) as I say is set for speed over stability, and will give you a faster connection in exchange for more disconnects.
