Try lowering your
rate
cl_updaterate
cl_cmdrate

until you have a stable connection but the game is still playable. Your "ping" in game isn't the network latency figure, but rather a collection of things including the latency. Try

rate 20000
cl_cmdrate 100
cl_updaterate 100

and work downward from these figures.
