What should happen is your account should be disconnected at the old address and transferred to the new one with your modem being disconnected.
If you go to
https://autoreg.autoregister.net/cgi/housemove and enter your username and password, attach the new modem and it should automatically activate and register the new modem. Should be as easy as that
