as far as I know you can leave these fields blank, you can put your name in the 'account' field, if you want, but the domain name can be left blank.
you should have
'require login' = no
a/c name & domain name - see above
IP address = get dynamically
DNS address = get automatically
MAC address = use default
Once that is set up a DHCP release/renew should get you an IP assigned.
I would recommend getting this working using only one pc directly connected to the router, before worrying about the wireless stuff ( which I know nowt about anyway

).
