It's certainly an issue that NTL are going to have to consider. The BW from BB+ will all be internal to their network, in the same way that calls from one NTL telephone to another are free, it would be nice to think that they could offer the BB+ service as outside the caps as the bandwidth costs are far more marginal to them.
