Call tech support at your earliest convenience and inform them that your modem keeps losing its' connection.
Mention if you like that you are seeing very high upstream transmit power.

Your downstream power is a little low but nothing to worry about, downstream SNR is a bit low too, however it's all probably part of the same fault. You need an engineer though as your connection will be somewhat intermittent.
