At work: Windows defrag (the boss can't see the point, and we would have to spend thousands on a corporate solution). Usually once or twice a week.

At home: Executive Software's Diskkeeper V. 9. I don't have a set amount of defrags a week, as I have it set up to run when the computer is not busy.
