the official line from ntl is that ntl customers will automatically get speed increases in the coming weeks and you need to do nothing about these upgrades however...
you can do it online by visiting the tier migration page....

or contact customer services and see who answers the phone
officially the csr's are advised that these speed upgrades are for new customers and that existing customers will automatically get the speed upgrades... however as you have noticed a lot of csr's seem to be upgrading customers anyway (which really is a bad customer experience as you have noticed with some being upgraded and some being told to wait
)
