Yes it is, unfortunately.

Of course we should wonder if this sort of thing has happened before but not received media attention. If ‘yes’ then that would suggest general incompetency by the prison service and ultimately the Justice Secretary.

If ‘no’ then fine it's a one-off, unfortunate to have happen in a case with so much public interest.

What do you think? Interestingly do you think anyone will lose their job (without compensation) over this? Thought not!
