Never been solely about "what do you have". It's always been about how does "what you have" affect you, and is it to a significant amount of discomfort. Eg Somebody showing very slight signs of osteoarthritis is not going to be considered in the same way as somebody needing a joint replacement. "What they have" is the same, but the impact is different.
