I wasn't sure if bandwidth for broadcasting the HD channels would have been a factor too, but I guess not.

It depends on the channel involved. Some, such as the shopping channels, pay to be on VM, some are cost neutral, such as the the BBC channels and some are paid by VM to be on the system, such as Discovery.
Again, the reasons why particular HD variants are missing vary. The channel may be in a contractual agreement to be exclusive on one particular platform for a length of time, such as ITV Encore HD, the channel owner may be literally or effectively refusing to allow VM access due to unreasonable carriage costs charged eg Sky Atlantic HD or it may be that both parties cannot agree carriage terms for a whole variety of reasons (including financial viability) eg MUTV HD.
AFAIK, VM don't have any bandwidth constraints that are preventing them launching any new channels as they have been upgrading their network behind the scenes.
