Very few people die from direct use of LSD.
LSD is actually one of the few drugs that does NOT cause physical harm, it is metabolized in your body and is gone before the effects have worn off.
There are a few exceptions though and people have died, but it's not being able to handle the mental state that LSD brings and can cause people to put themselves into physical danger (Examples: Walking in front of a train, car, trying to pet a lion, etc) that causes the deaths. If you're going to do LSD, stay in a nice room with nothing that can potentially scare you, and keep a "sober sitter" with you in case you think you may not be able to handle it.
FYI I have not done it for over 15 years, and no ill effects.
