If one month I didn't want HD, asked for it to be taken off for a month, would I then need to pay another activation charge every time I wanted it on/off through out my contract? Or do you pay a one off payment when you take out your contract and that lasts until you leave!

Yes you will need to pay install/activation charges. When you say
