It's called having higher population densities and laxer planning rules.
Don't kid yourself into thinking the rural areas in those countries have it any better than we do. You still need to live in a city for high speed services.

They can also string their cables up on poles which is far far cheaper, makes rolling out a network cost a fraction of what it does over here.
