If a restart fixed it there is a chance that your system was reading your shift key held down. Have seen it happen before though not since XP aside from one time in windows 7 when I enabled sticky keys by mistake.

As for the favourites thing, that has happened to me too though only seems to happen when I shut down my system without closing Firefox and even then rarely. Easy enough to restore if you look in the Firefox user folders.
