As I understand it, under the current Competition Commission rules, ITV's ad rates are regulated according to viewer ratings on ITV1. If ITV launch ITV HD and ITV1+1 these technically count as completely separate channels, even though presumably carrying exactly the same programmes and ads. If lots of people switch to them from ITV1, ITV will potentially lose lots of ad revenue.
The red button fix on Freesat presumably gets around this somehow (or numbers are too small to make a difference).
We won't be seeing ITV HD on either Virgin or Sky until this is resolved. May be fairly soon. See:
