Your downstream SNR is way too low. i.e. too much noise on the line. It needs to be 35 dB or above.

Maybe the coax cable needs screwing in tightly either end - or else it's an engineer job. It can be in the splitter into your house, anywhere up to the street cabinet or beyond.
