Are you absolutely sure about that? My mum & stepdad used to be on the 2Mb, & although they've now switched to the 10Mb, I'm pretty sure they got upgraded from 2 to 4 [for free, as part of the speed upgrade scheme] before that. Have any of the various VM-employees that are members here looked into it for you? I suppose it's possible your kneck of the woods could have managed to end up last on the list for upgrades, but if they've made the hardware changes necessary for the higher speeds like 50Mb as you mention, then it should be just a case of someone making the necessary adjustment to your account.

You could always switch to 10 before reading this thread then go back to 40.
If you're using Firefox I may be able to come up with a way to make that possible without having to go back into the Control Panel each time [e.g. a button or something]. [That is if I ever find out the syntax for using back-references in the Greasemonkey/Platypus extensions!

Speaking of posts per page, I actually used to have it set to 40, & switched to 20 because I got a bit impatient with how long it seemed to take for the page to finish loading. Whether that's just me having bad luck trying the CF server at busy times, or because the number of tabs I usually have open tends to be a 3 figure number
