It's nothing to do with the box doing anything wrong - I had the same thing when I ordered a couple of Prem Plus matches.
Because I ordered the game and then unplugged the telephone cable, the box didn't have time to send the signal setting the charge. As a result, when I put the cable in for a longer period of time, it sent the message saying that I owed for 3 matches (or however many it was). One quick call to Sky when I got the bill, they explained what had happened, so I happily(ish) paid up.
I agree that Sky should probably have mentioned an outstanding debt (although, if it's only from December, it's a recent bill anyway), the box just did what the box should
