Nope deffo every few seconds....
A few years ago, where I used to work, because our team was spread amongst several buildings, the boss installed a sort of diary system where we would all log everything we did. One of the technicians (bit of a Perl programming whizz) whipped up a small script that when run filled up the database that stored the diary with "Filled out job sheet" over and over. He got in trouble, but the diary idea was abandoned.
Back on topic. I personally don't see the point in jotting down all your thoughts. For most people, what you are thinking won't be relevant to them.
