I had nearly the exact same problem when I had BT (and ADSL) installed at my new house (no NTL in the new estate) and was initially offered £5 ish off. I then asked if this could be escalated to a manager as I was really not happy. They then reluctantly offered me my installation charge back.
INstallations does seem to be the worst thing for any of these comapnies to organise.
One of the ntl staff here will hopefully chase this up for you, or point you in the right direction of whose ear you need to bash!
