Agreed re: writing and sending recorded delivery.

However I would also copy in the MD of your region, or even the CEO at Head Office.

All too often, the reasons why people leave a service (any service, not just ntl) are never recorded and analysed. If you cite specific problems, issues and lack of resolutions, then at least it gives them the opportunities to address them for future and existing customers - even if they can't be bothered - at least you know you're doing the right thing.
