Keep it simple, state that the letter is formal notice of your intention to leave ntl, and that in accordance with the T&Cs you are giving a minimum of 30 days notice. Unless otherwise agreed between you and ntl in writing, the date of termination of services is to be xxx.

Make that last sentence bold for effect.
