Need Quick Help/Info...

Hi, I'm hoping someone can help.

My girlfriend wishes to cancel her ntl TV and phone services, but because her services have been restricted, she can't phone ntl, so I was wondering if it's a requirement to phone ntl to cancel? Or is there a certain address to write to?

If so, what is the appropriate address?
