Unless all the HD channels are going MPEG-4?! Surely that would cause them to have to deactivate them?

Unlikely -there are still many V+ boxes that are not MPEG4 capable.

Back but no sound on ITV HD it seems

Or Watch HD, Alibi HD, 5 HD, ITV2,3,4, Eden HD, Animal Planet HD

No Dave HD, Film 4 HD

Picture quality seems to have improved...

it's just BBC1 and BBC2 HD down, (994 is working as all are others that I've checked such as Eurosport, ITV HD no sound still)

BBC HD is back again, ITV HD still no sound. Sounds like the technical team at VM are having fun today.
