According to the OFCOM document, the information is contained in the program metadata provided by the broadcasters, which is then acted upon by the set top box.

The BBC still do it on some events like Trooping the Colour. But you need to go to the BBC iPlayer to get it. It used to be you could also get it on Red Button via 991. But the BBC have to justify the expense of broadcasting it now, hence catering for disabled people by providing AD gets priority.

TV must be locked down by the broadcasters, & soon we will have the “internet police” closing down anything that is even slightly “iffy”.
